UP Health Service

For students who are going to have face-to-face classes this upcoming semester, please follow these steps for your annual physical examination.

  1. First, acquire your case number or blue card (you can skip this step if you have one already). Please fill up the Kaalaman form and send to mrd-admitting.uppgh@up.edu.ph. Kindly wait for their response for your case number. Please take note they are only open from Mon-Friday 8:00 am to 3:00 pm only.
  2. Prepare the following diagnostic tests. These can be done at any diagnostic center of your choice. If you prefer these to be done at PGH, please contact 09617321764  for assistance in facilitating requests. Please accomplish this request form if you prefer your x-ray to be done at PGH.
    1. Chest Xray
    2. HbsAg,
    3. Anti-Hbs
  3. Accomplish PHEX form and obtain a signature from a licensed physician. You can choose from these options:
    1. Through UPHS consultants, students will personally come to UPHS for their PHEX
    2. Through a family physician. A teleconsult is needed for validation through UP Health service via OCRA for this option.
  4. After finishing the previous steps, please email results of your chest x-ray, HbsAg, and Anti-Hbs, and the signed PHEX form to hs.uppgh@up.edu.ph.
  5. Schedule final validation or clearance through Online Consultation Request and Appointment system (OCRA) Remember to choose the option PGH EMPLOYEE/ UP MANILA STUDENT so your request will be directed to the UPHS clinic queue.
