INSTRUCTIONS FOR ENROLLMENT for SECOND SEMESTER, AY 2022-2023
- Enlist through the Student Academic Information System (SAIS) using your credentials. If you can access your SAIS account but encounter problems during enlistment, please inform us by submitting your query in this link: https://tinyurl.com/SAIS-Enrollment-Concerns
- Fill out the Online Registration Form and submit enrollment requirements.

PhD Nursing: - Your registration adviser will validate your enlisted subjects and give approval. If your adviser has questions, he/she will get in touch with you directly through email. Expect correspondence from your adviser during the pre-enlistment and advising period.
- Your Form 5 will be emailed during the regular registration period (for BSN non-paying students only). For students who are not covered by free tuition and to all graduate students, instructions for payment will be emailed as well. The electronic official receipt (EOR) issued by the Cashier’s office will serve as your Form 5.

I have accessed my SAIS but it says that I do not have a valid enrollment appointment at this time. What does this mean?

I want to cancel the subject that I have previously enlisted. What should I do?
Write a request letter to withdraw or cancel your enlistment addressed to the College Secretary. If it is a Nursing subject, send your email to upm-cn-ocs@up.edu.ph with the subject line “Request for Course Cancellation”. If you wish to cancel a GE subject, send the request letter to upm-cas-ocs@up.edu.ph.
There are no more available slots in the subject I want to enlist in. What can I do?
You can request to add slot. For Nursing subjects, you may send an email to upm-cn-ocs@up.edu.ph with the subject line “Request to Add Slot”. For GE subjects, you may refer to their latest announcements posted on their FB Page at https://www.facebook.com/UpmCasOcs2016/
I want to enroll in a GE subject that is restricted to another program. What can I do?
You can enlist in these subjects after restrictions have been lifted and if there are still available slots. Please stay tuned on the date of lifting of restriction through the FB page of the CAS Office of the College Secretary.
I want to change sections. What can I do?
You need to accomplish the Change of Matriculation Form. This is usually encountered for subjects offered by the CAS. You may refer to their instructions posted on their FB page. For Nursing subjects, you may submit the request forms https://tinyurl.com/CNOnline-DocsRequest-Portal
The subject I enlisted in has been dissolved and I need to enlist in another section. What can I do?

I want to cross-enroll in another UP unit. How do I proceed?
Step 1: Download the Cross Registration Permit at this link: https://tinyurl.com/CN-Cross-Registration-Form
Step 2: Get in touch with your program adviser and seek approval.
Step 3: Submit the accomplished Cross Registration Permit at this link: https://tinyurl.com/CNOnline-DocsRequest-Portal
Step 4: The Office of the College Secretary will send back to you the form with the signature of the College Secretary.
Step 5: Send the form to the Office of the University Registrar for approval. Send it to upm-our@up.edu.ph
Step 6: Wait for the update from the OUR for your permit to cross-enroll with the signature of the University Registrar.
I want to go on a leave of absence. How do I proceed?
Step 1. Accomplished Leave of Absence Form (https://tinyurl.com/CN-LOA-form)
Step 2. Write a letter addressed to the Dean of the College of Nursing about the intention to go on leave and indicate the reason for filling:

I am coming from an absence without leave (AWOL) and I want to enroll again this semester. What will I do?
If you are coming from AWOL, you need to be readmitted into the program. Please follow the instructions posted on this website under Academic Policies & Procedures (please embed the link: https://upcn.upm.edu.ph/students/student-resources/academic-policies-and-procedures/)
I am on extension of my Maximum Residence Rule. When do I need to submit the requirements for the coming semester?

I am not covered by free tuition. How do I proceed with payment?

I have successfully enrolled already. When can I get my Form 5?
Your Form 5 will be emailed to you by the Office of the College Secretary starting 31 January 2023 onwards.
